Survey Templates

If you have multiple surveys with similar questions, you can design a base template you can reuse. This enables you to create a survey without having to add the same questions over and over again.

Getting There

To create a survey template, navigate to the Admin tab, and click Templates as shown below.

Adding a Survey Template

To add a certificate template, under Survey Template Settings, Click Add Survey Template.

Adding New Survey

To add a new survey to the survey template, fill in the following information.

Survey Name

Name the survey here. It will be displayed in the list of available templates.

Template

You can either create a completely new template by selecting No Template, or you can create a template based on an already existing template structure.

Click Create Survey to add a new survey template.

Editing the Survey Templates

Once you have created the template framework, click Edit to begin adding the questions and other details.

Add Info

Click Add Info to add a description to the page. Click update page to update the description.

Click Edit page info link to edit the description.

Adding New Questions

To start adding questions to the survey, click Add Questions.

Question Type

You can create two different types of questions for your participants to answer.

  1. Multiple Choice Questions
  2. Open-Ended Questions

Enter the question in the text box provided.

Add a Description

You can add a short description for the question. This can assist your participants in answering the question.

Multiple Choice

A multiple choice question type allows you to pick a suitable answer among a number of choices.

Enter the probable Answer Options that the participant would select.

You can enter and select multiple answers for a survey question.

Click Add Another Answer Option to add more answers.

Display Format

The display format allows you to decide whether you want your participant to choose only one answer from the list of options, or whether more than one answer can be selected. The display format also allows you to choose whether to allow the answer to be selected as a radio button (single answer), a checkbox (multiple answers), or a drop-down (single answer).

Set the Required Toggle

You can slide the toggle to make the question mandatory. Leave the toggle off if you want to make the question optional.

Click Save & Continue to add the next question.

Click Save & Close to stop adding the questions and exit from the dialog box.

Open-Ended Questions

Open-ended questions allow participants more freedom in providing useful feedback. A good survey often has a mix of multiple choice and open-ended questions, so you receive a good variety of answers.

Set the Required Toggle

You can slide the toggle to make the question mandatory. Leave the toggle off if you want to make the question optional.

Click Save & Continue to add the next question.

Click Save & Close to stop adding the questions and exit from the dialog box.

Adding/Deleting Pages to a Survey

If you want to add more layers of organization, or further edit your survey, you can always add additional pages or delete pages.

Click + Page on the left, to add pages.

Click Delete Page to delete an unwanted page.

You can only delete the first page of a survey if it has more than one page.

Click Next to proceed. Your survey template is now ready to be used when creating new surveys.

You can select a survey template when you are creating a new survey. Simply find and select it from the list of available templates.
